Since the house is in such a remote area, you will want to rent a car. www.autoeurope.com is a good source for Car rental while in Italy. They are located in Portland, Maine so transactions are USD. They are a middle man and not who you will rent from but have stellar reputation. I rented thru them the last time I was in Italy with no issues. My contact there is nicole@autoeurope.com . You should give her a call (1.888.223.5555) and see how her rates compare with other brands., mention my name. She has promised to give a discount when possible and terrific service. If you are flying into Rome and spending a few days there, you could train to Siena and get your car upon arrival in Siena. Il Poggiolo is only 27K from Siena.
Because I am there for over 17 days, I am doing a lease buy back thru Peugeot. It covers drivers 18 and older, all insurances necessary and anyone in the family can drive without being added as an additional driver.
An International drivers permit is a good idea, might not be necessary but...... American Automobile Association (AAA) is the place to get one. Here is a link to the application. You can do it thru the mail.
